Bound book, containing forms for sending for medical aid, completed by M. J. McCue, midwife of Moreton Morrell, Warwickshire (5 December 1977-6 September 1980), recording the name and address of the patient that the notice was sent in respect of, who medical aid was sought by, the date of confinement, whether the case was urgent, the name and address of the doctor or institution that the notice was sent to and the time the notice was sent.

M. J. McCue (fl. 1960-1990) was a midwife in Warwickshire.
